Live Scan Fingerprinting Service will be offered at the District Assembly Project Fair on Saturday, March 21st, 2026 from 10 am to 2:30 pm.
INSTRUCTIONS FOR COMPLETING LIVE SCAN SERVICE
A Service Provided for All Volunteers in All Rotary Youth Programs
For information about Rotary District 5150 Youth Protection Program or the need for fingerprinting, please click HERE or contact Youth Protection Officer David Dye (This applies to Rotarians, Rotaractors and/or adult community volunteers working on Rotary Youth projects)
STEP 1 – Register with the REGISTER NOW button on the upper right of this page and Pay the Finger Printing Fee $41.20 (includes Credit Card Fee) in advance to reserve a spot for you. If your club/sponsor agrees to pay for Live Scan fee, please contact your club/sponsor for reimbursement.
STEP 2 – Complete the request for live scan service form

Fill out the Live Scan form, print a copy and bring this document with you to the assembly.
Instructions on how to fill out the form:
Name of Applicant: Enter your Last Name, First Name, and Middle Name.
** Do not use initials or name abbreviations. **
Alias: Enter all other names you have used, including your maiden name.
Driver’s License No: Enter your Driver’s License Number, including the State.
DOB: Enter your date of birth (month/day/year).
Sex: Enter your gender (male or female).
HT: Enter your height in feet and inches.
WT: Enter your weight in pounds.
Eye Color: Enter the color of your eyes.
Hair Color: Enter the color of your hair.
Place of Birth: Enter your place of birth (City and State, or Country).
SOC: Enter your Social Security Number.
Home Address: Enter your residence address.
Your Number: Enter the name of the Rotary Club to which you are associated.
STEP 3 – Finger Printing service will be inside of District Assembly Project Fair from
10am to 2:30pm, Saturday, March 21st, 2026.
